Smith was born at Orangeburg Regional Hospital in Orangeburg, South Carolina, to Patricia Ann (née Smoak), an oncology nurse, and James H. Smith, a financial planner. When she was two years old, her parents divorced.

Smith started acting as a child and made appearances on stage in A Christmas Carol repertory from age eight to 11 and starred in a stage play with Richard Dreyfuss at age 15.
